Form 11
This form must be mailed by a Chapter to New Jersey DeMolay within ten (10) days following its
Installation of Officers.
New Jersey DeMolay Scholarships Form
This Scholarship application is open to all members who are currently high school seniors or enrolled in college or technical school. More details on first page of form.
Distinguished Service Award Application
The Distinguished Service Award (DSA) recognizes the most outstanding and inspiring members of DeMolay.

Sports Pre-registration Form
Use this form to register for any of our Sports competitions well ahead of the scheduled date of the event.
Sports Contact Form
Complete this form for your chapter and send it in to the Director of Athletics to ensure that your chapter
receives all the latest information regarding all athletic events.
Sports Medical Release
Every member under age 18 who wishes to participate in our athletic events is required to complete this form
along with parent/guardian signature.
Youth Protection Program Form
By completing this form, your chapter states that it has followed the guidelines for conducting the
Youth Protection Program as prescribed by the Executive Officer.
NJDAA Membership Application
Any Senior DeMolay or Master Mason may join the New Jersey DeMolay Alumni Association.
